HIPAA Privacy & Data Collection. I authorize Dr. J Anti-Aging Clinic to collect, store, and use the information I provide. My data will be transmitted to our HIPAA-compliant CRM (GoHighLevel) for the purpose of evaluating my candidacy for hormone replacement therapy and following up with me about my results. All data is handled in accordance with HIPAA regulations (45 CFR Parts 160 & 164) and will not be sold or shared with third parties outside of clinical care. I may request data deletion at any time by calling (407) 972-1197.
Age & Clinical Safety. I certify I am 18 years of age or older and that all information provided is truthful and accurate. I understand that hormone replacement therapy must be prescribed and overseen by licensed physicians, nurse practitioners, or physician assistants only. Pellet insertions must be performed in licensed clinical settings. Self-administered treatments at home are only permitted after receiving proper training and written authorization from a licensed provider. A formal in-person or telemedicine consultation is required before initiating any hormone therapy.
Dr. J Anti-Aging Clinic is a cash-pay practice and does not participate in Medicare, Medicaid, or any private insurance plans for office visits or treatments.
Florida Statute §458.3265. Lab work obtained through your own physician or insurance is always accepted.
